“Dear Providers,
With over 15 years of experience delivering home-based rehabilitation services, Visiting Therapists (also known as In Home Rehab & Wellness Associates) is proud to provide a specialized outpatient therapy model tailored for individuals with complex needs — particularly those enrolled in SCO, Care One, and Dual Eligible Special Needs Plans (D-SNPs).
We are a dedicated team of Physical and Occupational Therapy professionals committed to bringing the intensity and clinical quality of outpatient rehab directly into the home. Our services are designed for individuals who cannot access clinic-based therapy but require more comprehensive treatment than standard home health can provide.
Our model of care was originally developed in 2010 through our work with Commonwealth Care Alliance (CCA), where we served as a preferred therapy vendor. Since then, we have continued to expand our partnerships, delivering services to members under Tufts Health Plan, Blue Cross Blue Shield, and Medicare Part B. Our coverage area currently includes Somerville, Cambridge, Everett, Revere, Lynn, Merrimack Valley, Newburyport, Haverhill, and Lowell.
We work hand-in-hand with care management teams to support the goals of better access, improved outcomes, and lower costs. By coordinating care and proactively managing therapy and DME needs, we help reduce inefficiencies, prevent unnecessary hospitalizations, and improve member safety at home.
Every aspect of our therapy approach is patient-centered and results-driven. We bring portable outpatient-grade equipment and advanced therapy modalities into the home — enabling members to receive a level of care that’s both engaging and clinically impactful. Our team also includes Certified Lymphedema Therapists (CLTs) and DME-trained clinicians who support members with compression therapy, safety education, and proper equipment usage.
In addition to direct therapy, we provide comprehensive in-home assessments, including evaluations for Power Operated Vehicles (POVs), with full management of required PCP documentation and insurance authorizations. We also specialize in identifying and resolving duplicate or incorrect DME orders, coordinating with insurance providers and vendors to ensure accuracy, reduce costs, and support safer recovery at home.

Occupational Therapy offers comprehensive rehabilitation for individuals with functional limitations by engaging them in purposeful activities designed to improve their skills. This therapy also includes adapting tasks to suit individual abilities and making necessary modifications to the home environment, ensuring greater safety and independence.

How it works:
Step 1: Contact your Primary Care Physician
Contact your doctor and let them know that you’re interested in our services.
Step 2: Physician sends us an order/referral
Your doctor will need to send us a referral which will allow us to begin treatment.
Step 3: Insurance is verified for use
Our Office Manager will always check your insurance coverage prior to treatment so you will be aware of your patient responsibility, or out-of-pocket expenses.
Step 4: Scheduling of first evaluation
Your first appointment will be a full assessment to determine which services will benefit you and we will design a plan of care for you to meet your goals.
